Sixth Form Application Form
On 5th June 2025, Lancashire County Council’s Cabinet approved the proposal to close the sixth form provision at Moor Park High School and Sixth Form, with effect from 31 August 2026.

Moor Park Sixth Form is no longer accepting applications for year 12, however if you are looking for a main school place in year 7 please see the following link.
