April Update
As you will all be aware, we are unable to hold our drop in sessions for the completion of admission forms and checking of documents.
However, we are still able to provide you with all the necessary forms to complete at home which you can then either:

Required Documents
Passport
A copy of your child’s passport is required to ensure we have the correct name, date of birth and nationality on our system. It is also used to cross check with your child’s birth certificate to ensure all the information is correct.
Birth Certificate
A copy of your child’s birth certificate is required in order to crosscheck with the passport and admission form to ensure all the information we have on the system is correct.
Sending your Documents
You may take a photo of your child’s passport and birth certificate or scan a copy and email to admin@moorpark.mp with the rest of your forms.
Alternatively you may send a photocopy in the post with the rest of your forms to the address above.

Admission Form
The admission form is our main source of personal information regarding you, your child and your child’s emergency contacts. Please see the guidance document below for all the key information we require.
Medical Information
It is very important we have up to date medical information for your child in order for us to ensure they are cared for and safe in school, particularly with regards to asthma, allergies, diabetes or any other condition which may require your child to take medication in school.
- Medical Information Form – required for ALL students
- Allergy Declaration Form – only required if your child has allergies
- Asthma Declaration Form – only required if your child has asthma
- Medication in School – only required if your child may require medication in school.
Parental Consent for Sharing Images and Videos
This form is required in order for us to be aware of your preferences regarding the sharing of any images and videos of your child within school, on our school website, in our printed media or via our social media channels.
